The Youth Substance Use Service Survey information is protected under the Freedom of Information and Protection of Privacy Act. Per FOIPPA requirements, your information will be retained for at least 12 months after conclusion of data collection, after which time it will be deleted from our survey system.

To help plan treatment services in your area, you will be asked for the first three digits of your postal code. Any information obtained from individual respondents is used in a form that does not permit any particular individual to be identified. Your contact information will not be shared with anyone for any other purpose outside of this study. Your answers will be grouped with the answers of others who complete the survey so nobody will know who you are.

The data collected will be used to develop a plan for substance use treatment services for youth in the Simcoe County and Muskoka regions.
